Job Description Coram CVS/Home Health, an affiliate of Coram CVS/specialty infusion services, provides professional services in the home setting including nursing, physical therapy, occupational therapy, speech pathology and medical social services. As a Physical Therapist with Coram CVS you will establish treatment plans and goals that help restore function, improve mobility, relieve pain and prevent permanent physical disabilities of patients suffering from injuries, surgery or disease through a variety of modalities in the home setting.
In this role you will:
* Provide initial and ongoing assessment to determine level of function, including OASIS Assessment at appropriate time points.
* Responsible for planning, evaluation, and treatment implementation of assigned patients.
* Prepare timely clinical progress notes.
* Supervise therapy assistants as appropriate.
* Establish, measure and report on patient outcomes.
* Instruct patient/caregiver in the use of appropriate adaptive equipment and other assistive devices.
* Evaluate the patient’s level of function, and develop, implement, and revise an appropriate plan of treatment
with the patient, caregiver, primary nurse, and patient’s physician.
* Instruct the healthcare team, caregiver, and patient in the home therapy program and educate the patient and
family on the patient’s total physical therapy program.
* Provide patient and caregiver with written instructions, and communicate appropriately with the healthcare team
regarding patient’s condition, response to therapy, clinical progress, and changes in the program.
