Registered Nurse
As a Registered Nurse – Kidney Care Manager, you’ll be a member of the Care Management team, where you’ll deliver care and case management to chronic kidney disease and end-stage kidney disease patients. This position works closely with complex renal patients in their home, dialysis center, by phone, virtually, and electronically as needed. Work alongside other field-based clinical staff and physicians, non-clinical staff, and interact with providers, hospitals and ancillary services.
Nurse Practitioner
Join our team as a Nurse Practitioner, where you’ll provide care and case management for patients and deliver clinical care to chronic kidney disease and end-stage kidney disease patients. Work closely with complex renal patients in their home, dialysis center, by phone, virtually, and electronically as needed. This position serves as the clinical leader for the field staff.
Coram Home Infusion
Help patients receive Hemodialysis from the comfort of their home. We’re re-imagining kidney care, with a home-first approach that achieves better outcomes. Through our care management plans, we can help members manage their condition and prepare for treatment that fits their goals and lifestyle, including home-based dialysis treatment.
