Nurse Practitioner Fellowship with
Oak Street Health®, part of CVS Health®

The Oak Street Health NP Fellowship in Adult Primary Care is a 12-month paid program offered in partnership with the University of Michigan School of Nursing. It focuses on a value-based care model that emphasizes quality over quantity. Fellows gain the skills and support needed to become confident, competent clinicians for older adults. The program includes building a patient panel, learning from diverse preceptors, and engaging in advanced content that enhances—rather than repeats—graduate training.
 
Participants commit to working at Oak Street for at least one year after completing the program. Cohorts begin in September and March each year.

Program Requirements

To participate, you must:

  • Family, adult, or adult-gerontology primary care nurse practitioners who graduated or will graduate from their training program between December 2023-May 2025.
  • Currently residing in or willing to relocate to program location for at least the next two years.
  • Have not yet worked as an NP.
  • Minimum of one year experience working as an adult RN.
  • Minimum GPA of 3.0 in NP program.
  • Currently holds or will hold by 9/15/2025 an active, non-probationary state NP license.
  • A genuine passion for adult primary care.
  • A supportive attitude toward our patient population of older adults.
  • Embrace teamwork and the opportunity to collaborate with brilliant colleagues.
  • Want to be a part of an innovative model focused on empirically guided population health.
  • Spanish language skills are strongly preferred, depending on clinic location.

  • Please upload your CV using the link below. Once submitted, it will be reviewed by the Fellowship staff. If you meet the eligibility criteria, you will be invited to complete the full application. This next phase includes two short essays, the submission of three professional references, and supporting materials such as transcripts and applicable licenses.
  • We encourage prospective applicants to begin identifying references early. For the NP Fellowship Program, we require one (1) reference from a current or recent supervisor, and two (2) references from a Preceptor, Program Director, or Faculty member.

1. Didactic Learning: Fellows will have 4 hours of didactic learning on a weekly basis, 2 hours are asynchronous and 2 hours are in live learning sessions with other fellows. These sessions are led by fellowship faculty and other subject expert matter. We have over 80 different learning topics!

2. Preceptorship: The Program provides our fellows with real-time clinical feedback and direction in the form of precepting. 

3. Faculty Advisors: Fellows will have bi-weekly sessions with a faculty advisor and other fellows in their cohort to support their professional development.

Behavioral Health Fellowship with
Oak Street Health®, part of CVS Health®

Oak Street Health’s Master-level Behavioral Health Specialist training program in the revolutionary collaborative care model in value-based setting, which rewards healthcare companies for the quality of their care, not the quantity. 

This 12-month paid training program will provide all the necessary skills, support and learning required to become a highly confident and competent clinician for older adults, while providing you clinical supervision hours to give you a pathway towards becoming a clinical licensed social worker.

Each fellow is paired with a clinical supervisor for obtaining clinical hours with the goal of becoming a licensed clinical social worker.  They will earn clinical supervision hours from a licensed clinical social worker and participate in fellowship mentoring to enhance their graduate education knowledge. The fellowship year will end with the presentation of a Capstone Project aimed at bridging the collaborative care model theory and practice.

Program Requirements

To participate, you must:

  • Family, Adult or Adult-Gerontology Social Worker who have already graduated or will graduate from their training program in the near future.
  • Willing to work in the assigned location for the program duration.
  • Minimum GPA of 3.0 in MSW program.
  • Currently holds or will hold by the fellowship start date, an active, non-probationary state lower-level (associate-level) social work license in the state they will be working in.
  • Genuine passion for behavioral health (done well, the practice of short-term solution focused therapy should be fun).
  • A supportive attitude toward our patient population of older adults.
  • Embracing teamwork and the opportunity to collaborate with brilliant colleagues.
  • Want to be part of an innovative model focused on empirically guided population health.


Selection Timeline

  • First-round interviews will be conducted as applications are received.
  • Second and third-round interviews will be virtual or in person (based on applicant preference) as individuals are moved forward from the first-round.
  • Final decisions will be delivered once the candidate has been selected. 


Checklist
Prior to submitting the application, please ensure that you: 

  • Meet all the admission requirements
  • Have included all the required attachments:
  • CV with current work history and detailed  information on your social work internships
  • Completed application
  • MSW Diploma (if available)
  • Unofficial graduate school transcript
  • Copy of current Social Work license (if available).
  • If applicable, non-US residents must provide a copy of their permanent resident card/VISA/proof of eligibility to work in the US.

The program will include time spent building your knowledge of evidence-based interventions and psychotropic medications, earning clinical supervision hours from a licensed clinical social worker, and engaging in didactic content that aims to build on, not repeat, graduate training. All fellows will work at Oak Street for 24 months following the completion of this program, but we hope will spend their careers at Oak Street improving the health of communities across the country.  The program has two concurrent cohorts starting in September and March of every year.

Fellows will learn from a variety of teachers, including Social Workers, Psychiatrists, Psychologists and PhD’s. The program will include exposure to a variety of specialty services provided by Oak Street, including Problem Solving Therapy, Behavioral Activation, Psychopharmacology, the Collaborative Care Model and others.
